1980s Exorcist Rip-Off Film with Car Crash

I saw this film on video in the early eighties - scared the hell out of my young self.

All I can remember was that it was an Exorcist rip-off, and that at the beginning and the end we see a small child playing with a toy car, but the toy car represents a real car which we see crashing (perhaps caused by the child).

The child might have been on a boat, but I'm not sure. I think I can also remember a young girl vomiting slime much like in The Exorcist.

If anyone can tell me what this film was called I'd be be really grateful.
