Boy Chokes At Dinner Table, Father Killed By Toy Monkey

I'm trying to remember the name of an old movie that opens with a boy choking to death at the dinner table. The father takes a knife to cut open his son's throat and the boy dies.
It is a ghost story that ends with the father's throat being cut by a toy monkey holding clanging cymbals.
