Crazy Surgeon Saws Off Arm and Teases Students With It

I remember watching this film with my grandmother when I was really little where a large, jovial guy us giving some type of presentation to a group about a saw and he uses it to cut off the arms of a patient (it may have been one of the men in the group or a corpse).

He walks around the group (they are in a circle, with the body in the middle) and uses the severed arms to start massaging a woman's shoulders.

In another scene, a woman is running or driving from him and goes to a drive through and it's him on the intercom.

Does anyone have a clue what this might be?
