Guillotine, Doll, Dumbwaiter and Mrs. Garrett-Type

What's the film takes place in a dark room where there's a doll(s), a guillotine that chops their heads off, and possibly a dumbwaiter or staircase?

The dark room is in a quaint house with a garden and the old lady in the film reminds me of Mrs. Garrett from 'The Facts of Life'.

I believe it had a long title with the words 'Mrs.' and/or 'summer' or 'summerwind' in it.

I do hope someone can figure this out - it's been haunting me for years...er....you know what I mean =D
