Horror Show Episode About Nurse Fleeing From Sequined Creature in Woods

Does anyone remember this late 60's or early 70's horror show (Creature Feature or Chiller) episode?

A nurse in a white uniform is running through woods and is suddenly being chased by a sequined (lady) creature. She (the nurse) trips on a log and cannot run anymore. The sequined monster goes to the house in the woods to find the patient whom the nurse was taking care of.

This episode has stayed in my head since I was eight years old. Can you help?
