Man in Cell Attacked by Tadpoles and Later in Butcher Shop by Pig-People

I've been trying to find this movie forever.

A man is in a cell a few inches above water and there's other people in different cells telling him not to touch the water, which he does. Then these creepy tadpole things crawl into his skin and through his body.

Another part I remember is the man trying to escape this dungeon-looking butcher shop with a bloody tub in it. Pig-like people are trying to kill him and he hangs one on a hook. He also pushes one and it's head slams into a hook and dies.

Toward the end of the movie, the man wakes up and he's in an examination room with all these wires and tubes in his head, as if it was all a dream.

Help please.
