Man Kills Famly And Props Them in Front of TV

Looking for a horror movie about a family man who for some reason kills his family and rounds them up in front of the TV in the living room.

He then goes away and is involved in car crash, after which he wakes up in a hospital, his eyeball hanging on a thread. He recovers slowly and pretends he is a normal guy interested in the nurse. She falls in love with him during his recovery but he is planning on killing her. She finds out in the end, and he chases her.
