Man Tries to Burn Article Causing VooDoo Curse in '80s Film

I once saw an early '80s voodoo film where people are dying one by one.
One character is heading home to destroy the article behind the curse. On the way, he passes a car being pulled from an accident scene and drives on not knowing it is one of his loved ones who has died in the crash.
Upon getting home and setting a fire to burn the article, or maybe the house, he trips and falls into the fire and dies himself.
Any ideas?
