Old Woman Gets Axe to Head While Watching TV

I'm trying to find a horror movie! It's an early 90s maybe late 80s movie I watched when I was little....

The parts that I remember best are an old woman watching TV in her home sitting in a chair and she gets axed in the head.

Then there's another scene where a school bus driver was driving down a dark neighborhood and a small dog (mixed breed/mutt) was in the middle of the road. I think at one point the bus drivers eyes bulged out of the sockets.

Please help me find this movie!
