Old Woman With Two Faces Tries to Play Music Box

I'm looking for the title of this old horror movie where an old woman with two faces is trying to play a music box. I'm assuming she is a mutant or some sort of a monster.

If anyone has the slightest clue as to the title, I'd very much appreciate it! Thank you experts!
