Searching for 70's Frog-Like Creature With One Massive Eye

In the late 70's, I was a young lad, inspired by my father's amazing artwork.
I remember seeing Dawn of the Dead at the drive in with him at age five. It was awesome.
On to my question. My dad drew in pencil this huge, frog-like creature from a horror movie jacket and put it on my wall. The creature had one eye considerably larger than the other and he would put a glow-in-the-dark frisbee in place of the eye. It scared the crap out of me.
Shortly after, my dad gave up his art and now at age 35, I have given up my job to pursue a career in art. I would love to redraw this creature for my dad as a gift. The problem is I don't know the title of the film, and never saw it. Any help would be greatly appreciated!
The creature on the cover of the VHS tape looked much like a frog. It seemed to be squatting with legs to the sides, arms in front. The limbs may have been thin. The key to this picture is the massive eye. I believe the creature had both eyes, but for whatever reason, one was large (enough for a fricking frisbee). I would guess there was little else on the cover, but it has been so long.
I've included a quick sketch. I could have tried to put more detail in, but don't want mislead anyone.
