Sewer Monster Chases Man in '70s Movie

Sometime in the mid '70s, I stayed up way past my bedtime and watched part of this movie on TV where a man is being chased through a sewer by a Black Lagoon type creature. He tries to escape up a ladder only to find the manhole cover at the top has a car on it.
The monster has caught up to him by then, and he evades it by pushing himself behind the ladder and sticking the monster with a pen or something pointy to make it go away.
