Short Horror Stories From a Comic

Watched this 20 years ago. Not sure what comes first but the details are here. Can anyone tell me what movie this is?

The main scene were a kid reading a horror comic, can't remember where he gets it from, maybe from the rubbish bin.

Scene started with the kid flippin' the comic page then the view pans nearer and nearer to the comic page and the picture turns into reality (you know, the classic stuff), so the short story started.

The first story if i remember correctly has 2 doctors and a body on the operating table. Probably doin' some investigative work on the body to know how he died or somethin'. Not sure what happen in between, when the doctor cut open the body, not sure which part, cockroaches pours out and attacked the doctors, some scene I remember was a cockroach bites out from the body inside out, probably went in the doctors body and bite its way out, the flesh flips open and cockroaches comes out. Then the story ends by turning reality shot into a comic strip again, then the camera pans out i guess..

Another story the same way as the first one, camera panning in the comic book page and turn into reality and the movie starts. This one I remembered some people went into some cabin in the jungle probably, hiking fishing or whatever, the same stuff as "Cabin Fever". They're there for some vacation or somethin'. Someone was celebratin' his birthday there, he got killed by some monster and the monster decapitate his head and put on top of his birthday cake, he then bring the cake to his girlfriend probably, and sings happy birthday song, this monster looks like "Ghost Rider", big, skeleton head (I think), wears like the Terminator (haha). After that the same as the first story, it turns into a comic page again and pans out.

Don't remember if there's anymore stories but in the end, we see the kid continue to read the comic and he got caught by his father and his father is not happy about it, took the comic and throw it in the rubbish bin, his son obviously is unhappy about it, he took out a voodoo doll and vigorously poke nails on the doll's neck, then we see his father holding his neck screamin' in pain.
