Skeleton Follows Woman in Mirrors

I'm trying to remember the name of this film or TV show episode where a woman sees a scary skeleton behind her every time she looks in a mirror or anything reflective.

She goes crazy and covers all reflective surfaces in her apartment, including the faucet in her tub. Then, one day she decides to look in the mirror again and when she does, she sees herself as the skeleton and jumps out the window.
